Certificates of Training

Each year departing residents receive certificates indicating the length and scope of residency or fellowship training.  Name and medical degree will be listed on the certificate based on documents provided during credentialing that indicate the degree awarded by medical school. An “MD” will not be substituted for “MBBS”, etc. unless official documentation of MD conferral by New York State is presented. Click here for details on the NY State Office of the Professions MD conferral process. The certificates are issued at the request of the resident's training program and only for the period of approved training satisfactorily completed.

• Certificates are issued twice each year: June and December
• Certificates list deadline for issuance in June is March 1st
• Certificates list deadline for issuance in December is October 1st
• Duplicate or replacement certificates can be ordered for a fee of $40.00 which includes a certified mail fee.  A notarized letter explaining the reason for the request is required.

 

A check or money order should be made payable to SUNY-HSCB and submitted with the letter to the GME Office:
