No specimen will be accepted without a properly completed Surgical Pathology Protocol Form (SP-1). It is necessary to write clearly on a Form #SP-1. One form per procedure is required regardless of the number of specimens submitted. Also, all specimen containers submitted must have a completed label affixed to it.
