General Information

DownstateCards are only issued where the person requires ongoing and frequent access to Downstate facilities over a long period of time. In cases where the service involves only limited INFREQUENT and OCCASIONAL access of SHORT duration or where an individual (such as contractor/vendor/ staff) changes frequently, the individual needs to obtain a one-day visitor pass from University Police each time they come to DMC. The University Police office is located at the University Hospital entrance on Lenox Road.

The DownstateCard office continues to process ID cards for students, paid/non-paid staff and each of the affiliated categories as detailed below:

Official Paid State Employees

The State Human Resources (DHR) office issues the card authorization document for all new paid appointments. The new appointee will bring this document to the DownstateCard office to receive their new card.

Official Research Foundation Employees

The Office of Research Administration (ORA) issues the card authorization document for all new paid appointments. The new appointee will bring this document to the DownstateCard office to receive their new card.

Official Unpaid Appointments

This includes State voluntary academic faculty appointments as well as Visiting Faculty appointments. Persons who have official unpaid appointments through the Department of Human Resources should already be in the DownstateCard database. You can visit the campus website at to search the personnel phone directory to check if a person is in Personel Database System.

New Official Unpaid Appointments

The State Human Resources (DHR) office issues the card authorization document for all new official unpaid appointments. The new appointee brings this document with them to the DownstateCard office to receive their new card.

Affiliated Categories

These include the specific entities and programs below of persons who do NOT receive a regular paycheck from either the State or Research Foundation. Persons may be paid by one of the affiliated DMC entities listed below or receive payment for services as a vendor/contractor. These individuals will receive a LIMITED ACCESS type or TEMPORARY type card that has an expiration date printed on it depending on the nature of their engagement and their access needs.

Affiliates include the following specific programs identified to date with the associated program coordinator:

  • Alumni Association
  • Animal Care and Use Committee appointees
  • Arthur Ashe Institute for Urban Health, Inc.
  • Attorney General's Office
  • Chaplain's Office.
  • Childrens Center of SUNY Brooklyn, Inc.
  • Clinical Practice Plan employees
  • Clinical Practice Plan subcontractors
  • Continuing Medical Education Program
  • Contractors: Persons who are paid by an official Downstate Contract or Purchase Order, or from one of the official affiliated corporate entities listed below. These include contractor staff such as SODEXHO employees, construction-related subcontractors, consultants, etc. The Program Coordinator (the Downstate office which supervises the contract, program, or activity) is responsible for providing verification/documentation for all covered persons in that program/contract/activity
  • Vendors: Persons who are sales representatives of outside companies such as pharmaceutical and medical equipment companies. This category is required to pay a $20.00 non-refundable fee for initial ID issuance and subsequent replacements
  • Visiting Students
  • HSCB Council Members (President's Office)
  • Library Subscribers (Library Administration)
  • Spouses/Family Members of Resident Students
  • Visiting Faculty
  • Volunteers
