How to Download the ID Authorization Form

1. Click on the link below, the form is a PDF file.
2. Fill out the form on the screen.
3. Print the form on your department's official DMC stationery and have the department head or program coordinator sign the form.
4. Give the form to the individual to submit when they come to the DownstateCard office. Only original signed forms will be accepted; forms not printed on official DMC stationery will not be honored.
