Onboarding Instructions for Accepted Pathway Students

 

The list below contains detailed instructions for students who have already accepted their invitations to participate in our summer pathway programs. It is not for prospective applicants or those who have not received an acceptance letter.

Required materials may vary for students even in the same program so please refer to your premetric checklist in the blue menu bar of your application for your required documents and deadlines. Failure to complete the necessary documents will delay your enrollment.

Early Medical Education (EME)

 

W-9

A W-9 form must be completed and signed in order for you to receive a stipend for your participation in the program. Please make sure that the information is correct so as not to delay the processing of your payment accounts. Although the payment will be set up for direct deposit, whichever address you put on the document needs to be the address that a check can potentially be mailed to.

Document must be uploaded to your application via the Document Upload section located in the blue menu bar.

  • The W-9 must be typed.

Model Release

SUNY Downstate Health Sciences University celebrates the achievements of our students, faculty, and staff.  In order to respect and protect the privacy interests of the University’s students and employees on campus and at institutional activities, the University has implemented a photo release policy. Throughout your time with us we may take images/photographs and/or videos of students, faculty, and staff on campus or participating in school activities. These images/photographs and/or videos may appear in various Downstate publications, including, but not limited to, the Downstate website, newsletters, programs, brochures, institutional calendar, social media, or mass media outlets. The model release form provides all persons interested/not interested in having their image/photograph and/or video presence captured and used for marketing, communications, and PR purposes the opportunity to opt in or out.

Data Access Form

In order to provide students with effective programming the Office of Diversity Education and Research requests access to student academic records to track their progress in the program and after program completion. In compliance with the Federal Family Educational Rights and Privacy Act of 1974, information from your student records are prohibited from being provided to a third party such as information on grades, billing, tuition and fees assessment, financial aid (including scholarships, grants, work-study, or loan amounts) and other student record information. You may, at your discretion, grant the Office of Diversity Education and Research with permission to access your student records through your educational institution and/or the National Student Clearinghouse by completing an authorization to Access Student Education Records form.

Health Clearance

SPRINTER: Only students who will be doing in person research must complete a health clearance form

All students who will participating in our summer programs in-person/on campus. Must be medically cleared by our health center before they are allowed to begin.

HIPAA

The Health Insurance Portability and Accountability Act (HIPAA) made individually identifiable health information “protected” (hereafter “protected health information” or PHI).  As a result, anyone wishing to use or disclose PHI must adhere to the requirements outlined in HIPAA’s Privacy Rule and as modified by The Health Information Technology for Economic and Clinical Health Act or “HITECH”.

All diversity program students involved in research or patient interactions at SUNY Downstate must complete appropriate training on the viewing and handling of PHI.

To access your training, you will receive an email link to complete an on-line tutorial on the Health Insurance Portability and Accountability Act. These forms are federally mandated and will arrive with a due date. If you need more time to complete the training, please let us know.

Once you have completed your training you save a copy of your completion certificate and upload the document to your application via the document upload section located in the blue menu bar.

Failure to complete the training will delay your enrollment process.

CITI Training

CITI (Collaborative Institutional Training Initiative) is the online course that ensures that people participating in any aspect of Human Research are familiar with the core principals and ethics of Social/Behavioral research. You will need to complete the CITI training for your research internship. The courses should take you at least 7-10 hours to complete.

After completion of the course you must save your completion report” and “completion certificate” to upload into your application via the document upload section located in the blue menu bar

Failure to complete the trainings will delay your enrollment process.

Official Transcripts

A copy of your official transcript from ALL U.S schools attended is required.

Please have transcripts sent electronically to diversityprograms@downstate.edu.  

Study Skills Assessment

The Study Skills Assessment is designed to identify your academic strengths and weaknesses. Completing the assessment is the first step in putting yourself on the path to academic success as you help yourself become a better student!

To complete the assessment, you will receive a link via email for the questionnaire.  We will be notified of completed questionnaires by the academic advisement team.

Results of the assessment will be given during your summer here at SUNY Downstate by the Academic Advisement Center along with tips and strategies for improvement.

Testing Solutions

Testing Solutions is an MCAT test prep company that specializes in the Critical Analysis and Reasoning Section (CARS) of the exam. This will be your main source of CARS prep in the EME program. To begin you will receive an email from Testing Solutions with your registration information. After you have been fully registered, you must complete the following items by the due date on your pre-matric checklist:

  • Vocabulary Diagnostic
  • CARS Diagnostic
  • 5 hour Recorded Fundamentals Video

This is the start of your CARS tutoring and it is crucial that you complete all diagnostic exams and review the video in its entirety and understand the concepts being taught.
