Supplemental Application

All applicants must complete a SUNY Downstate Supplemental Application in order to have their application considered for admission. Once we receive your verified AMCAS application, an email invitation will be sent to your email address on file with AMCAS granting you access to the Supplemental. We recommend that you complete the Supplemental and pay the processing fee (or submit an approved AMCAS fee waiver), as soon as possible.
