HIV Prophylaxis Following Occupational Exposure

SUNY Downstate currently follows the guidelines of the NY State DOH AIDS Institute. It is preferable to begin the following prophylaxis (updated October 2012) as soon as possible (within 0-2 hours) following exposure, and no later than 36 hours following exposure:

Truvada (emtricitabine 200 mg and tenofovir 300 mg) once daily
AND
Isentress (raltegravir 400 mg) q 12 h

If source patient is HIV positive, PEP should be taken for 4 weeks.

Documentation

1. Healthcare worker: Baseline HIV, Hepatitis C Ab, Hepatitis B Surface Ab, CBC, Comprehensive Metabolic Profile.
2. Source patient: HIV (counseling and consent forms must be filled out before specimen can be submitted to lab), Hepatitis C Ab, Hepatitis BsAg, etc.
3. It is the hospital's responsibility to provide the appropriate follow-up of the source patient. If the incident occurs at KCH or UHB, and there is a problem with your post-exposure care, you may page the Infectious Disease fellow on call. If he/she does not answer, page the Infectious Disease attending or, if needed, page Dr. Michael Augenbraun, Division Chief of Infectious Diseases.
