MyDownstateCare is your secure, online portal that connects you to your health care information and your Downstate medical team.
In your portal you can:
Records for patients aged 0 through 12 months and 11 through 17 years are ineligible for portal access.
For urgent medical matters do not use MyDownstateCare to send any messages requiring urgent attention.
For medical emergencies please dial 911.
Create a FollowMyHealth patient portal account through an email invitation
Once the clinic registrar enters your email address at registration, we send you an email invitation with a personalized link from FollowMyHealth. Do not share this link with others.
Before you begin
Create a username and password which will be your FollowMyHealth username and password.
The FollowMyHealth section is identified by this symbol:
Note: Alternatively, you can enter a username other than an email address.
Your credentials are immediately validated. If the guidelines are not followed, a red "X" and a message are displayed in a red bold font, indicating the guideline that was not followed.
Note: You must accept the terms of service to use the patient portal.
Create a FollowMyHealth patient portal account and connect to another patient account as an authorized portal user
If you assist dependent patients, such as your parents or children, with aspects of their healthcare, consider becoming an authorized individual with access to their FollowMyHealth® account.
Before you begin
Request an authorized portal user invitation by sending an email to MyDownstateCare@downstate.edu. After verifying your identity, we will email an invitation.
Sign in with FollowMyHealth username and password
After you register and have a FollowMyHealth username and password, you can sign into the FollowMyHealth® Patient Access portal.
Click Sign In.
Recover your username from the patient portal sign-in page
Use the Forgot your username link on the FollowMyHealth portal sign-in page to recover your FollowMyHealth account username.
Your identity is verified with a text or email message. A code is sent through a text message to the mobile phone number on the account or an email message is sent to the email address that is associated with your account.
The mobile phone number must be a verified number. If you do not have a verified phone number with your patient portal account, the text message option is not available.
If you do not have access to text or email messaging, select I don't see an option that will work and contact technical support.
If you do not receive a code, or need a new code, click Resend verification code or What if I didn't receive a code. A code is resent up to three times. If you do not remember your code the third time, your account is locked for five minutes.
A page opens with your username.
Your email address becomes your FollowMyHealth username. Enter a password and click Continue to sign in to the patient portal.
Check for an email verification notifying you that your username is recovered or created.
Reset your FollowMyHealth password
Reset your FollowMyHealth password from the FollowMyHealth® Patient Access portal sign-in page.
Your username is required to reset your password. If you forgot your username, see the link, "Recover your FollowMyHealth username."
A message indicates that a verification code was sent to the email address associated with your FollowMyHealth username.
If you do not receive a code, or need a new code, click Resend verification code. A code can be resent up to three times. If you do not remember your code the third time, your account is locked for five minutes. An email message notifies you when you make the maximum number of attempts to resend the code or enter the correct code.
You are signed in successfully to your FollowMyHealth® Patient Access portal account. Check your email for a message notifying you that your password is changed.
Notifications are sent to you through an external email address or mobile phone. The email address that you used when registering for a FollowMyHealth account is shown in the Communication Preferences tab, the Demographics page in My Info, your FollowMyHealth preferences, and the Goals tab in Wellness.
If you change the email address in one of these locations, the change is made at all locations.
Note: The Goals tab is displayed in Wellness when your provider has the goals feature enabled and you have accepted a goal.

A verification code is sent to the new email address.
If you do not receive a verification code, click Resend verification code.
If you have a connected organization and provider to whom to send demographic updates, send your demographic updates with your email address changes to one or more of your connected organizations. All available organizations are displayed alphabetically and are selected by default.
If you are the parent or legal guardian of a patient who is under the age of 11, you can request access to your child’s health information in the FollowMyHealth® patient portal.
Authorized Portal User Access enables you to view some of your child’s health information in the portal. When your child turns 11, you will not have access to any new information in your child’s portal. Please email mydownstatecare@downstate.edu to request access.
FollowMyHealth is an online tool that gives you anywhere, anytime access to your personal health records. This allows you to take a proactive role in managing your care.
