REIMBURSEMENT

TUITION

Please fill out a Tuition Waiver Form and bring it along with an itemized tuition bill to our office. Your bill should show the class name and number of hours per class. You will receive a Tuition Waiver worth $1000 to present to the Downstate Bursar to apply towards your tuition.

BLS

Please read the BLS Reimbursement Instructions and follow the instructions on filling the forms out and what paperwork to bring.

Please note that reimbursement is available only to those who fall under Nursing.

CONFERENCES

Please fill out an Approved Absence Form and have it approved and signed by your Nurse Manager.
Upon completion of your conference, you will need to bring in your receipts for all aspects of reimbursement you are claiming - registration, and if applicable - flight, hotel, transfers/transportation and meals.
A standard voucher will be created for submission to Accounts Payable. Reimbursement will come in the form of either a physical check or direct deposit into your bank account.
