Instructions for Completing and Returning the Consent Form

Signing the Document

Saving Your Work
- Once completed, go to File > Save As (or Download if using a browser).
- Crucial: Please save the file with your name included in the filename (e.g., ConsentForm_FirstName_LastName.pdf).
- Note: If you are using a web browser to fill the form, ensure you download the "filled" version, not the original blank template.
Delivery
