High Risk Neurodevelopmental Follow up Program

- 2nd and 3rd Thursday of the month
- 8-12 am- Televisit; 1-4pm- In person

Patient Criteria:
- Gestational age ≤ 34 weeks
- Birth weight less than 1800grams
- Chromosomal anomalies
- Hypoxic Ischemic Encephalopathy during the perinatal period
- Any abnormal neurological exam, neonatal seizures, IVH, or CNS infection
- Others: Infants not included in these criteria but identified to be at risk by the physician
Schedule of Visits:
Children will be referred directly from NICU at discharge or by the primary care Pediatrician. Care will continue from 4-6 months corrected gestational age through 5 years.
1st appointment: Televisit, under 4 months
2nd appointment: 4-6 month corrected gestational age;
3nd appointment: 11-13 months corrected gestational age;
4rd appointment: 15-18 months corrected gestational age;
5th appointment: at 2 years chronological age;
6th appointment: at 3-4 years chronological age;
7th appointment: at 4-5 years chronological age.
When necessary, patients will be seen more frequently.
Televisit: 1st appointment under 4 month corrected gestational age, and follow up
In-Person: Subsequent in person developmental testing
