• 2^nd and 3^rd Thursday of the month

Patient Criteria

- Gestational age ≤ 34 weeks

- Birth weight less than 1800grams

- Chromosomal anomalies

- Hypoxic Ischemic Encephalopathy during the perinatal period

- Any abnormal neurological exam, neonatal seizures, IVH, or CNS infection

- Others: Infants not included in these criteria but identified to be at risk by the physician

Schedule of Visits

Children will be referred directly from NICU at discharge or by the primary care Pediatrician.  Care will continue from 4-6 months corrected gestational age through 5 years.

1^st appointment: Televisit, under 4 months

2^nd appointment: 4-6 month corrected gestational age; 

3^nd appointment: 11-13 months corrected gestational age;

4^rd appointment: 15-18 months corrected gestational age; 

5^th appointment: at 2 years chronological age;

6^th appointment: at 3-4 years chronological age;

7^th appointment: at 4-5 years chronological age.

When necessary, patients will be seen more frequently.

Televisit: 1^st appointment under 4 month corrected gestational age, and follow up

In-Person: Subsequent in person developmental testing
