What is Healthy Aging?

Healthy Aging is the process of developing and maintaining the functional ability that enables wellbeing in older age. Functional ability is about having the capabilities that enable all people to be and do what they have reason to value (WHO).

With aging, our bodies goes through multiple changes that can affect our ways of living, but it does not mean you have to stop enjoying things and activities you love most. By staying healthy and active we can effectively impact the way we age.

Our BRIDGE Team is dedicated to promote and teach our affected population and anyone who is involved in care for them about Healthy Aging and ways to achieve it. Our goal is to help our elderly population to stay healthy, be active physically and socially, and to have full and meaningful lives.

Project SCENE

As part of the BRIDGE program and to promote healthy aging we have developed Project SCENE (Senior Citizen Educational Needs Endeavor). The mission of Project SCENE is to empower elders to make informed health decisions, to prevent disease, disability and injury among the elderly, and to promote healthy behaviors for the aging population. BRIDGE provides diverse programming for Brooklyn's Elders, including:

  • Community Education and Outreach: in-person and online educational programs focused on the healthcare needs of Brooklyn's elders. We provide culturally-informed, interactive workshops on: Using health technology to improve quality of life, Interacting with the healthcare system, Preventive Care, Disability, Fall Prevention, Caregiver Issues and Stress Management, Mental Health, Polypharmacy, Healthy Aging, Insurance eligibility/enrollment, Alzheimer's Disease and Related Dementias - treatment and care. We also provide educational programs for community organization staff and community healthcare workers.
  • Health screenings and health fairs focused on the screenings and referrals for: Blood pressure, Cholesterol, Glucose, Height and Weight, Body Mass Index, Staying Strong- Fitness and Readiness, Dental, Vision, Glaucoma, Prostate Cancer (DRE-PSA), Dementia, and Depression.
  • Patient-Centered Engagement and Education Programs (PEEPS): In addition to our standard community education programming, we will be implementing engaging, drama-based health education programming within the community and our clinic settings over the coming year - stay tuned!
