Update on our MRSA Accomplishments

As you know, we have begun our WE CARE journey to become a highly reliable organization (HRO).  Zero harm to patients is one of the goals of our HRO journey.  Many of you ask, “Is zero harm really achievable?”  Can we totally eliminate hospital acquired infections, pressure ulcers, falls, and other unplanned incidents that patients currently suffer? The answer is twofold; we need to try and we are on our way.

For the past year, there has not been a single hospital acquired methicillin resistant staphylococcus aureus (MRSA) infection at UHD.  We would like to commend our Infection Control Team for implementing anti-MRSA initiatives at UHD to drive these results at the hospital. This team worked very hard to get these projects off the ground and we appreciate their committed effort.

The initiatives are as follows:

- Isolation precautions for all patients with MRSA isolates

- Active surveillance for MRSA in the ICU population (enforcement of nasal swabs for all admitted to the ICU)

- Hand hygiene adherence

- Monitoring and timely feedback

- Reinforced CHG bathing for ICU patients, patients with central venous lines, Foley caths, and patients on contact precautions

Congratulations to our Infection Control Team for leading these initiatives, the multidisciplinary teams who ensured implementation, and the ongoing active surveillance to maintain compliance. It truly takes everyone working together to improve our MRSA stats for the hospital! These results prove we can become a HRO and be recognized as the ‘Best Place to Get Care and the Best Place to Give Care.’

In conclusion, I want to encourage everyone to take some time off this summer. Connect with family and friends you have not been able to see over the past two years. Enjoy the outdoors and everything New York has to offer. Let us enjoy the summer and take some time to relax and decompress safely.

Announcing our Health Home Program

University Hospital at Downstate is teaming up with Brooklyn Health Home (Maimonides Hospital Center) to unveil our Health Home Program in our Emergency Department. Health Home is a Program for Medicaid patients who meet the following criteria: Serious or persistent mental health concerns, one chronic condition and at risk for a second chronic condition, HIV/AIDS, and sickle cell anemia.

Our program was developed with the help of Ms. Felicia Thompson, LMSW, MPA, Director of Social Work, Carol Rowe-Johnson, MSA, BSN, CCM, Manager of Case Management, Ms. Susan Fraser-McCleary, RN, LMSW, CCM, Assistant Vice President for Care Coordination, and the Emergency Department’s Medical and Nursing Leadership team. The goal of the Health Home Program is to make sure patients get the care and services they need. This can lead to fewer trips to the emergency room or less time spent in the hospital for our patients. It could also mean getting regular care and services from doctors and providers.

This program is about healthcare and service providers working together to make sure our most vulnerable patients get the care and services they need to stay healthy. Each Health Home enrollee will have their very own care manager. The care manager helps set up a care plan, appointments, and provides access to the services needed to assist with better health for the patient.

Some of the services include:

- Healthcare providers

- Mental health and substance abuse providers

- Medications

- Housing

- Social services (such as food, benefits, and locating transportation)

- Other helpful community programs

Let’s put our hands together for this amazing and necessary program for our community!

This past May 8-14, we kicked off National Hospital Week with a blast and showed our appreciation for everyone at the hospital and our clinical sites. Many of you really did strut your stuff down the ‘tapis bleu’ as we celebrated our clinical staff for another successful year.

We appreciate every one of you for your dedication to excellence in patient care as we continue with our WE CARE journey and become a High Reliability Organization!

Look at a few pictures of our team below from our multi-site events that also includes our Paint-N-Sip and the Family Friendly Feud game!

Employee of the Month for May – Maryann Barbera

maryann_barbera

Maryann Barbera, one of our Social Workers in the Department of Social Work, strives to achieve excellence in everything that she does. She is dedicated to her patients and the entire team. She can be found after hours on many occasions documenting in the EMR, ordering Durable Medical Equipment (DME), or visiting patients. Patient care is her top priority.

Ms. Barbera contributes to our team by developing a positive work environment through interactions with her team, laughing, making jokes, and being the support person that lifts everyone’s spirits when needed. Always optimistic, she will take on extra responsibility before being asked for assistance and volunteers to help others complete their assignments.

Our team notices Ms. Barbera’s dedication and commitment. We are all very proud and appreciative that she is here at Downstate. For this and many other reasons, we are ecstatic for Ms. Barbera and her accomplishments here at UHD.
