University Hospital at Downstate is teaming up with Brooklyn Health Home (Maimonides
Hospital Center) to unveil our Health Home Program in our Emergency Department. Health
Home is a Program for Medicaid patients who meet the following criteria: Serious or persistent mental health concerns, one chronic condition and at risk for
a second chronic condition, HIV/AIDS, and sickle cell anemia.
Our program was developed with the help of Ms. Felicia Thompson, LMSW, MPA, Director
of Social Work, Carol Rowe-Johnson, MSA, BSN, CCM, Manager of Case Management, Ms.
Susan Fraser-McCleary, RN, LMSW, CCM, Assistant Vice President for Care Coordination,
and the Emergency Department’s Medical and Nursing Leadership team. The goal of the
Health Home Program is to make sure patients get the care and services they need.
This can lead to fewer trips to the emergency room or less time spent in the hospital
for our patients. It could also mean getting regular care and services from doctors
and providers.
This program is about healthcare and service providers working together to make sure
our most vulnerable patients get the care and services they need to stay healthy.
Each Health Home enrollee will have their very own care manager. The care manager
helps set up a care plan, appointments, and provides access to the services needed
to assist with better health for the patient.
Some of the services include:
- Healthcare providers
- Mental health and substance abuse providers
- Medications
- Housing
- Social services (such as food, benefits, and locating transportation)
- Other helpful community programs
