This product has been approved by the P&T committee for use in neonates and infants unable to take hydrochlorothiazide tablets.
The dose is 10-20 mg/kg/dose every 12 hours
Pharmacists and nurses please ask prescribers to change hydrochlorothiazide orders for doses less than 12.5 mg or fractions of doses less than _ of the 25 mg tablet to chlorothiazide.
For example 3.5 kg baby 2 mg/kg/dose hydrochlorothiazide would be 7 mg.
This dose should be changed to chlorothiazide 70 mg susp (20 mg/kg/dose).
Prescribers for discharge prescriptions
Chlorothiazide suspension 50 mg/ml
Dose 70 mg (1.4 ml)
Every 12 hours po for X days
Dispense (2.8ml x # of days supply)
Since this is a commercial product and does not require refrigeration or additional preparation by parents it should be readily obtainable and less troublesome than using hydrochlorothiazide tablets for these patients.
