Instructions for New PatientsYour appointment should be at least 3 days before the beginning and at least 3 days after the end of your next expected menstrual period. A proper evaluation is not possibly in the presence of menstrual flow. Use NO antibiotics or antifungal agents for at least 7 days and preferably 14 days before your appointment. Do not douche or use any intravaginal medications for 7-14 days before your appointment. Oral contraceptives should be continued, as should any medications you are taking for nongenital indications such as heart disease, duodenal ulcer, hypothyroidism, etc. If you use spermicidal jelly or cream for contraception, either abstain from intercourse or have your partner use a condom for 7 days prior to your visit. Initial visits require about one hour. Follow-up visits require about 1/2 hour. Please try to arrive on time. FEES If it is necessary for you to cancel an appointment, please do so AT LEAST 48 HOURS IN ADVANCE so that we can reallocate the time. You may be billed for appointments which you do not keep or which you cancel at the last minute without a reasonable explanation. INSURANCE INFORMATION I participate in most but not all insurance plans, including Medicare, GHI, Oxford, Empire Plan, Cigna, 1199, Aetna, Health First, Health Plus and Americhoice. If your plan is not listed, please call your insurance carrier to determine if I participate in your plan. If you belong to an HMO, or you are not sure of your type of plan, please call your insurance company. It may be necessary for you to obtain a valid referral (recognized by your insurance plan) from your primary physician for a visit to the specialist.
