The Healthy Living Partnership (HLP) is a joint effort between the SUNY Downstate Community, the New York State Department of Health (DOH) and the Center for Disease Control (CDC) in collaboration with American Cancer Society (ACS). The purpose is to promote early detection through comprehensive screening for breast and cervical cancer at its earliest stage when treatment is more effective. Q #2 Who is eligible for HLP? Women 40 years and older who are uninsured or underinsured are eligible for Clinical Breast Examination (CBE), Mammogram and Cervical (Pap Smear) Examination. Women 18 years and older may receive Breast Self Examination (BSE) Instruction, CBE and cervical examination if they are sexually active. Q #3 How do I make an appointment? If you or someone you know is interested in the HLP, please call 1-718-270-8846/8245. An appointment is usually made after the Screening Intake Form is completed. Q #4 How long is the wait for an appointment? The average waiting period for an appointment is approximately weeks or depending on the availability of space. Q #5 How will I get my result for the mammogram and pap smear? The Mammography Department will notify your health care provider and send you a letter with the mammogram findings within thirty days. The physician/nurse practitioner will send you a letter regarding the pap examination findings. The CCHPW staff will also calls you as a follow up. Q #6 What if I receive a bill? Measures are taken to ensure that you do not get a bill. However, if you get a bill, do not pay the bill or ignore it, call the CCHPW immediately so that the problem can be investigated and resolved. |
