The same form is available in two formats Adobe 'pdf' and Microsoft Word, whichever you find easier::
Shipment Discrepancy Report/ Shipping Authorization Form in Word format: Users with Word software can complete the form, then 'save as' to a new unique document name, then attach and send in an e-mail to Central Receiving.
Shipment Discrepancy Report/ Shipping Authorization Form in Adobe pdf format. Users with Adobe Professional Version software will find this easier to complete (tabs from field to field), save then e-mail to Central Receiving. Users with Adobe Acrobat Reader version software will be able to complete the form online, but then you will need to print off and fax the form to Central Receiving (X-1266). If you bring your package to Central Receiving, the computers at Central Receiving have the Adobe Professional Version software that you can use when you arrive to complete the form online.
